CAQH Contact Information is Changing on May 19, 2014. Please Update Your Records. CAQH has grown substantially over recent years with the launch of several new initiatives. To accommodate this growth, CAQH is moving into new office space. Effective May 19, 2014, the CAQH address, phone and fax numbers are changing.
